Its western terminus is at State Route 134 nearly 4 miles (6.4 km) southeast of Martinsville, and its eastern terminus is near the unincorporated village of Torch at the concurrency of U.S. Route 50, State Route 7, and State Route 32 in extreme southeastern Athens County.
